Need some wiring help!

I need to know where to place the lead and the ground wires on this old Gibson setup.  I'm using it with a Dark Star pup.

The control panel is set up, from left to right in the picture:  jack,  tone and volume.

Any ideas?

I found the EB-0 schematic on Fly Guitars, so we're set (I hope!).
