Many years ago I had the best tech I've ever known tell me that tube defects from the factory are extremely rare. Shipping issues provide the majority of defects. Of course this was long ago when tubes were made in the USA.

Well, at least it was rare that faulty tubes left the factory.
That is not the case nowdays, bad tubes that should be thrown away leave the factory and end up in amps, sadly..
