IIRC, they didn't know if the stories they were hearing about him beating her were true so they warned him that they better not be. When they were confirmed, he was tossed out. It was huge deal because he was also in Kyuss, QOTSA's predecessor.

Am I correct in remembering that heroin use was also his problem?

I heard rumors that both he and his then-wife were using and that's what the fights were over, but I've also heard that refuted.
