Can anyone ID this old Ampeg preamp?

I haven't seen one of these before, and Google draws a blank. 

Maybe it's been rehoused?

That looks like one of the extremely rare first edition SLM SVP preamps, which is basically just the preamp section of the old SVT (the later 12AX7 version), tubes and all. They only made them for a short time.

Wow, never saw one of those. At first I was thinking someone cut the face out of an MTI-era blackface SVT. Kinda nifty that it's got both channels.
